This includes direct service delivery (face-to-face) or indirect (resource development). The duration of each session is up to the discretion of the therapist and the child’s representative and can be adjusted to meet the child’s needs and environment. The minimum charge for the duration of each session is 45 minutes. A typical session is 1 hour duration.
This is charged according to the Modified Monash Model (2019) – refer to https://www.health.gov.au/resources/apps-and-tools/health-workforce-locator/health-workforce-locator and review travel categories.
A minimum of 15- minutes (round trip) per session is charged for areas within MM1-MM3. 45-60 per session is charged for areas located in MM4 and above. This is determined by the therapist’s discretion with consideration of shared travel to other participant’s location surrounding appointments.

This may include the development of reports requested by the participant’s representative for school staff or treating team. More extensive reports may be additional costs. The reports are completed indirectly, some requiring some face to face observations or parent completed questionnaires as part of the assessment.
This report outlines the OT goals being worked towards. It specifies smaller short term goals as a means to achieve the longer term goals and a list of strategies that will be used to reach these these goals. This document is useful to be completed within the first 2-3 sessions to ensure goals and service provision are consistently reviewed and all services within the team are working collaboratively alongside one another.
It requires 1 hour indirectly for report writing.
This letter is useful when families require guidance in purchasing items under their core funding (within their NDIS plans). It is a requirement for purchasing higher cost items and can be used to support individuals if ever audited by the NDIS.

The NDIS Progress Report identifies the goals being worked towards, progress made, limitations of service provision or events impacting on the implementation of service provision, as well as recommendations and the plan forward. It's useful for the individual's NDIS plan review to support funding required for their next plan.

The Sensory Profile Report is used to identify an individuals sensory needs and recommendations for strategies, environmental adaptions or equipment that will support the individual within different contexts to enable their independence. The Sensory Profile Assessment Questionnaire is completed alongside observations completed by the therapist to further evaluate the findings.

The Functional Assessment Report is useful in explaining how independent an individual is and how much support they may require. This is typically used when being assessed for NDIS funding or when reviewing how much an individual may has differed to their previous level of function. The questionnaire assessment is compared with observations to further evaluate the findings.

The OT report is a combination of the Functional and Sensory Profile assessment, providing the reader with an overview of the individuals independence and level of function holistically. Recommendations are also included to support the findings.
